'Long-term conditions or chronic diseases are conditions for which there is currently no cure, and which are managed with drugs and other treatment’
On occasions, there’s a requirement for a longer-term approach to addressing home care and support needs. This may be due to having a long-term condition such as motor neurone disease or Parkinson’s disease, or due to a physical disability that reduces levels of independence. At myCare we know our clients prefer to remain in their own homes rather than having to go into residential care and can provide the solution to enable this.

How Might I Be Affected?
A long term condition may mean you are unable to independently manage activities of daily living such as washing and dressing, or managing household tasks.
How Can We Help?
We can help you with the tasks that you are no longer able to manage independently. We can sit down and discuss your needs and how we could help to retain your independence.
How we can help:
- Providing personal care and support such as washing and dressing as required
- Fetch shopping and/or prescriptions
- Domestic support in the home such as cleaning or doing the laundry
- Social support such as visiting for a friendly chat
- Support with bills and other paperwork that may have gathered whilst you have been in hospital
- Provide respite care for a couple of hours or longer if required
